Final report in 2012 baby death received by HSE

The HSE has confirmed it has recieved the Final Report in relation to an investigation carried out following the death of a baby boy at Cavan General Hospital following child birth over a year and a half ago.

After the incident the Area Manager for HSE Cavan & Monaghan, commissioned an external review to examine this adverse outcome. An Independent review team were commissioned from the National Maternity Hospital, Dublin to undertake this review and the thrust of the investigation was to establish the factual circumstances leading up to the incident, while also identifying any 'key causal factors' that may have contributed to the loss of the baby.  

The HSE has confirmed in a statement released on Friday afternoon that the Final Report, which was due at the end of last month, has now been received.  

'Hospital Management would like to acknowledge the significant time and efforts of the review team in the completion of this independent report and they give a commitment that it’s recommendations will be addressed in full', a HSE spokesperson said.

Representatives from the HSE have met with the baby’s parent’s and have again expressed their regret for the loss they have experienced. The parents have been furnished with the final independent report and they have been given a commitment that the report’s recommendations will be addressed.  

The HSE says it also acknowledges the 'trauma' staff of the hospital have undergone.  

'We request the family's right to privacy at this time and we will be refraining  from any further comment while they take time to consider the report '.
